FOOD INSECURITY QUESTIONNAIRE

The purpose of this questionnaire is to gather information about patients who have inadequate access to food. This information helps to identify patients who are as a result of food insecurity at increased risk of complications related to surgery. This allows us to direct patients to resources to help them with inadequate food resources.

INSURANCE AND BILLING

ATACS accepts all insurance plans, including private health insurance, Medicaid and Tricare. We are preferred providers through Aetna, Blue Cross Blue Shield, Cigna, Moda and UHC.
